Job Title: Hospice RN Visit Nurse

Location: Greater Twin Cities Metro (21 County Service Area)

Salary: $90,000 – $130,000 DOE

Schedule: Full-Time

Position Summary:

As a Hospice RN Visit Nurse at Superior Hospice, you will play a vital role in delivering high-quality, personalized care to individuals navigating their end-of-life journey. Your nursing expertise, empathy, and presence will bring comfort and dignity to patients and their loved ones during their most vulnerable moments.

Responsibilities:

  • Patient Assessment: Conduct comprehensive assessments to understand each patient’s needs and develop individualized care plans
  • Symptom Management: Manage pain and symptoms effectively to improve patient comfort and quality of life
  • Emotional Support: Provide compassionate support to patients and families, building trust and understanding
  • Education: Empower families and patients with information to help them make informed care decisions
  • Collaborative Care: Partner with interdisciplinary team members to ensure cohesive, whole-person care
  • Documentation: Accurately document assessments, care plans, interventions, and progress per hospice standards
  • Family Liaison: Serve as a trusted point of contact between families and the care team, promoting unity and clarity throughout care

Qualifications:

  • RN Licensure: Active Registered Nurse license in Minnesota
  • End-of-Life Expertise: Solid understanding of hospice and end-of-life care principles
  • Compassionate Approach: Genuine empathy and passion for supporting patients and families during sensitive times
  • Communication Skills: Excellent verbal and written skills to ensure clear, compassionate communication
  • Clinical Competence: Strong clinical and critical thinking skills to meet complex patient needs
  • Cultural Sensitivity: Respect for diverse cultural, religious, and personal beliefs to provide personalized care
  • Adaptability: Ability to navigate dynamic and emotionally complex situations with professionalism and grace
