Applewood Rehab Center is seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality patient care in a skilled nursing and rehabilitation environment. The ideal candidate brings strong clinical skills, professionalism, and a commitment to supporting residents with compassion and attention to detail.
Key ResponsibilitiesProvide direct nursing care under the supervision of an RN or physician.
Administer medications and treatments safely in accordance with facility policies.
Monitor, assess, and document patient conditions; report changes promptly.
Support individualized care plans and coordinate care with interdisciplinary teams.
Maintain accurate clinical documentation and follow all regulatory guidelines.
Assist residents with daily living needs and provide emotional support.
Uphold infection control and patient safety protocols throughout all activities.
Active and unrestricted LPN license .
Previous experience in long-term care or rehabilitation settings preferred.
Strong communication and critical-thinking skills.
Ability to work collaboratively with nursing staff, physicians, and support teams.
